Through these plans, you will enjoy quality
coverage at competitive group rates. These options are a great
solution for individual members or small businesses that do
not have access to group voluntary dental or to members employed
by large companies that do not offer a quality dental insurance
solution. To enroll, you must be a present member of the FAMB,
or an employee of a member of FAMB covered in this plan. Dependent
coverage is available to lawful spouses, domestic partners,
and unmarried dependent children under age 25. |

PPO Dental Plan |

This dual option plan, through Ameritas, allows
you to receive the maximum benefit and lowest out-of-pocket
costs by visiting an Ameritas PPO network dentist, but also
gives you the flexibility to visit any licensed dentist you
wish. As low as $13.08/month. Quarterly entry dates of January 1st., April 1st., July 1st., October 1st.

DeltaCare Co-Pay Dental Plan

This plan offers the convenience of
a fixed copayment schedule through a panel of carefully
selected dentists in your area. The plan requires
you to select a dentist from the DeltaCare network
list. EyeMed Vision Care included. As low
as $12.82/month. Monthly entry dates, 1st of each month.

Focus Vision Plan |
| The Focus Vision plan, offered through Ameritas,
features the VSP nationwide network. The plan provides coverage
for eyecare needs such as exams, lenses, contact lenses, and
