Member Group Dental/Vision Plans

These voluntary dental plan options were designed specifically for members of the FAMP and their employees. As a member of FAMP, you are part of a large group with buying power.

• No minimum participation requirements

Through these plans, you will enjoy quality coverage at competitive group rates. These options are a great solution for individual members or small businesses that do not have access to group voluntary dental or to members employed by large companies that do not offer a quality dental insurance solution. To enroll, you must be a present member of the FAMP, or an employee of a member of FAMP covered in this plan. Dependent coverage is available to lawful spouses, domestic partners, and unmarried dependent children under age 25.

PPO Dental Plan w/ Optional Vision

This dual option plan, through Ameritas, allows you to receive the maximum benefit and lowest out-of-pocket costs by visiting an Ameritas PPO network dentist, but also gives you the flexibility to visit any licensed dentist you wish. As low as $16.16/month. Quarterly entry dates of January 1st, April 1st, July 1st, and October 1st
